Healthcare Equity with Molina Healthcare

Faith in Action of the San Gorgonio Pass provides Enhanced Care Management (ECM) services through Molina  to support individuals and families with complex health and social needs. ECM is a Medi-Cal benefit that provides intensive care coordination and community-based support for people who are experiencing homelessness, chronic health conditions, behavioral health challenges, or other barriers to stability.

Our ECM program focuses on health equity, ensuring that individuals who face the greatest barriers to healthcare and stability receive personalized support, advocacy, and connections to services. We work directly in the community — not just in an office — meeting clients where they live, stay, or receive services.

Transportation Services

Faith In Action provides assistance with bus passes and gas cards to people who participate in one of our programs and making progress towards self sustainability.

Community Resource Events

Our FIA Community Resource Events are aimed at disseminating information related to economic well-being and self-sufficiency in the community.

Each Wednesday from 10:30am – 1pm,  Faith In Action opens the door to our office at 66 S San Gorgonio Ave Suite #6 to provide food , clothing , shoes, and hygiene products to those in need.

Medi-Cal Outreach & Enrollment

Our Medi-Cal Outreach Program helps community members apply for, renew, and understand their Medi-Cal health coverage. Many individuals lose coverage due to paperwork issues, renewals, or changes in income — our team helps prevent coverage gaps and ensures individuals and families stay connected to healthcare services.

Food Access & Assistance

If you are in need of food, contact your local food bank to find a pantry near you. Information about a few resources can be found below.

Employment Assistance

Through our Job Prep Bootcamp and Life Skills Program, participants will learn about job-seeking skills and have access to computers to continue their job search with assistance from our Navigators.

Street Outreach & Mobile Outreach Van

Faith in Action (FIA) operates a Mobile Outreach Van that brings services directly to individuals and families who are experiencing homelessness, living in rural areas, or facing barriers to transportation and healthcare access. Instead of expecting people to come to an office, our team goes into the community to meet people where they are — on the streets, in encampments, in motels, in parking lots, at food distributions, and in underserved neighborhoods.

Our mobile outreach program focuses on building trust, providing immediate assistance, and connecting individuals to long-term services that improve health, housing stability, and overall well-being.

Criminal Defense Services

Once a month Faith In Action (FIA) hosts lawyers from the Riverside Public Defender’s office. Services include:

  • getting a warrant recalled
  • probation terms
  • removing a Driver’s License hold
  • expungements
  • anything else related to criminal law.
